Front has blank fields at the top left for return address and is printed at the center with "NEW YORK TELEPHONE COMPANY / P.O. Box 70, Inwood Station, New York 34, N.Y." Top right is printed with "398-MBX" Back of envelope is printed below flap with a cartoon of a woman seated on an ottoman at the foot of a bed, speaking into a corded phone held in one hand and holding a feather duster in the other hand. A cat lays on the ground at her feet." Underneath the flap, where the envelope would be sealed is printed "PLEASE ENCLOSE YOUR BILL STUB". On top of the flap is printed "EXTENSION PHONES IN COLORS!
