Catalogue entry no. 426 by Max Loehr:

426 Small Dragon with Bird-headed Tail
Light gray-green and opaque grayish brown jade with a dull surface finish. The dragon’s head, with a finely cross-hatched crest and muzzle, is unusual in regard to its closed mouth. Other uncommon features are an appendage forming a loop at the animal’s neck and a long, wing-like projection in front. The tail terminates in a small bird head with a cross-hatched crest. The body is covered with plastic spiral curls. Perforation at bottom center. Late Eastern Chou.
